What: U11-U19 Boys & Girls competitive teams; 5 guest players allowed for competitive

Where: Valley Ridge Soccer Complex, Cedar Hill, TX

Team representative or Manager will need the following for check-in:

A copy of their roster signed by the registrar and coach
Team Check-in Sheet. XLS or PDF
Notarized medical releases for every player including guest players
Guest Player Release Forms - DOC or PDF
7 copies of the misconduct game report. Must be TYPED. (XLS or DOC)
A USYSA coach’s ID card (provide by your home association)
